PLA application for funding
Have the following documents ready to submit your PLA application for funding.
- ✅ Your valid Passport or Birth Certificate
- ✅ Driving Licence or Utility Bill
- ✅ Non UK passport holders will also require an eVisa Share Code/EU Settlement
Please note: Applications which do not contain all required documentation may be rejected and additional applications for funding would be required.
Employed? You'll need:
- Last 3 months payslips (end of each month if paid weekly)
Self Employed? You'll need:
Dated document showing initial registration with HMRC as newly self employed within the current financial year.
Or
Most current full SA100 HMRC self-assessment Tax Return with submission receipt .
Alongside
Completed Self Employment declaration form attached (guide to where this is available from)
Carers Element? You'll need:
Full-time Carers (paid or non-paid)
- Attendance Allowance Letter (AA)
- Carers’ Allowance Letter of Award
- Disability Living Allowance Letter (DLA)
- Personal Independence Payments (PIP)
- Armed Forces Independence Payments (AFIP)
- Valid Blue Badge
